Issues raised in public, followed up in public
Local Health Accountability and Advocacy Teams (LHAATs) gather evidence at facility level and take it to the people who can act. This page tracks that advocacy: the visits, requests and commitments made, and where each one stands. Changes the LHAATs have confirmed at follow-up are reported under Results and Impact.

From a facility visit to a verified change
Issue raised
A LHAAT visit or scorecard documents a gap: for example, a broken water pump, or no delivery service after 4pm.
Response secured
The LHAAT takes its evidence to the office that can act, and records what that office agrees to do.
Followed up
The entry stays on this page, with its status, while the LHAAT follows it up at its next monitoring round.
Verified or escalated
When the LHAAT confirms the change on a follow-up visit, it moves to Results and Impact. Issues that stall are escalated.
Local Health Accountability and Advocacy Teams
Each LHAAT scores its facility monthly on vaccine availability, cold chain, staffing, malaria commodities and maternal services, and sits across the table from the LGA and state officials who can act. Teams are community members, ward development committee representatives and health workers, supported by the consortium.
Tone and verification
The tracker records what was raised, what was promised and where it stands, in plain language. Entries name offices and roles, not private individuals, and nothing is reported as a result until the LHAAT has confirmed it at follow-up. Corrections can be requested through the contact page.
